New Patients
Northwest Oncology & Hematology, S.C. has been serving adult patients with
malignant diseases and blood disorders since 1978. We welcome you into our
practice and appreciate the stress that frequently accompanies a visit to an
oncologist. We will attempt to schedule you with the physician to whom you have
been referred, and to do so as soon as possible. If he or she is not available
at the time you wish or need to be seen, one of our other equally qualified
associates will gladly assume your care. We do ask that you personally bring
along copies of your past medical records at the time of your appointment. We
are mostly concerned with lab testing, x-ray reports, surgical procedures, and
biopsy reports. You may be requested to bring copies of the actual x-ray films;
these can be obtained from the facility where the studies were performed. Our
office will be happy to assist you in obtaining any of your information.
To Schedule a New Patient Appointment:
Call our New Patient Coordinators at (847) 577-0620. If you want to make an
appointment in Rolling Meadows or Barrington, call ext 286. If you want
to make an appointment in Elk Grove or Hoffman Estates, call ext. 308. You may be asked
to leave a message. Please leave your name and phone number and available times
to reach you. You will receive a call back by the end of the business day.

your request for an appointment to our new patient coordinator. You will receive
a call back by the end of the business day.
New Patient Coordinator Hours:
8:30-4:15 Monday - Friday
Be sure to bring your insurance card to your first visit.
Many of the required forms you will be asked to complete at your first visit are
available here for download and printing. They may be completed prior to your
arrival and hand carried to your first visit.
